Thank You, Thank You.  We do not know how to adequately say thank you for your time and efforts.  It is because of your time and efforts that we have been able to help our citizens live independently and safely in their homes. 

The efforts of our volunteers have made it possible to enrich and better the communities we live in.  All of your efforts and the time you put in are greatly appreciated.  Again thank you.

Below you will find some information to help you and hopefully make the efforts you go to for us a little easier. 

You can now report your hours online.  This will mean no more time sheets for you.  Just click on the link below.

Forms:

Registration Form - pdf - Just print it out, fill in your information, and return it to us.

Time Sheet - pdf - Just print and record your hours and return it to us.  You can also just click above and report your hours online.

Update Sheet - pdf - Use this when you just need to let us know you moved or a new phone number or have a new volunteer station or job.  Or anything else you just want us to know.

Transportation Log - pdf - This is a form for those who transport others to medical appointments and errands. 

Current Newsletter - pdf - This is our most current newsletter.  Trip information, upcoming events and birthdays are listed along with other interesting facts.

Information:

Handbook - pdf - The Handbook provides additional information on the way our program works.  You can also save this to your computer.  It includes instructions for reporting your hours as well as a list of stations.

Insurance Brochure - pdf- This is a sheet that explains the insurance every volunteer has supplied from us.  It is in addition to your own insurance.  You don't have to pay for this, it is free.
